People other providers have exited

Behaviours of concern, substance use, a history nobody has documented. If the referral has been knocked back twice, it's worth sending here.

Health complexity at home

Wound care, post-surgical support, chronic conditions where somebody needs to notice the change and act on it rather than log it.

Plans that need somebody to push

Housing applications, equipment funding, allied health that has been sitting unactioned. We'll chase it and tell you where it got to.

How a referral runs

Five steps, and the first answer comes inside a day.

    Referral received

    Send it through with the difficult parts included. We'd rather hear those first than find them in week three.

  2. 02

    Contact within 24 hours

    We contact the participant or the referrer within one business day, with a straight answer on capacity and fit.

  3. 03

    Meet and greet

    Everyone meets before anything is signed. The participant confirms it works for them, not just for the plan.

  4. 04

    Support planning

    A support plan built together, around what the person actually wants their week to look like.

  5. 05

    Supports commence

    Services begin, with the same worker each shift and updates that arrive without being chased.

Response time

Within 24 hours.

Business days. If we’re mid-shift it goes to the answering machine, and we call back between clients rather than letting it sit.

A nurse was needed. The role had been advertised on a portal and left there. One phone call, and somebody was booked within two hours.
We’d rather do the coordination ourselves than watch somebody wait for it.

Communication

You won’t be chasing us.

  • Notes and invoicing happen on a fixed day each week, not whenever there's a gap.
  • Anything that changes a participant's risk, health or housing gets a call the same day.
  • If an approval comes through, the family hears about it from us straight away.
  • If capacity changes, you'll know before it becomes your problem.
