One household, two systems
NDIS and aged care, under one roof.
Plenty of South Coast households have an NDIS plan on one side and a Home Care Package on the other. A wife and a husband. A parent and an adult child. Usually that means two providers, two rosters and explaining the same household twice.
We cover both, with the same person walking in the door.

The chasing is covered
Equipment funding, referrals, approvals. Both systems generate paperwork and both get followed through rather than logged.
It scales with the household
If a second package comes through later, there is already somebody in the door who knows everybody.
How it usually starts
It starts with one person, and then the house changes.
A couple near Ulladulla came to us through their son. She was the participant, and we had been with her a long time before anything else changed.
Then her husband’s health shifted and he came on under his aged care package. Same worker, same week, one conversation with the family instead of two.
Nobody had to start again, and nobody had to explain their own household to a stranger at the worst possible moment.

Home Care Package support is delivered under Trilogy. The NDIS side is direct, as a registered provider. Each is claimed and reported against its own funding.

How it works in practice.
Can the same provider deliver both an NDIS plan and a Home Care Package?+
Yes. They are two separate funding systems with their own rules, but there is nothing stopping one provider working across both. Care Awareness South Coast is NDIS registered and delivers Home Care Package support under Trilogy.
Does the funding get mixed up?+
No. Each side is claimed against its own plan or package and reported separately. What is shared is the person doing the work and what they already know about the household.
What if only one person in the house has funding right now?+
That is common, and it is often how it starts. Plenty of households add a second package later as things change, and having somebody already in the door makes that far less disruptive.
Do you cover this across the whole South Coast?+
From Nowra and Jervis Bay in the north down to Batemans Bay, including Ulladulla, Milton, Mollymook, Narrawallee, Burrill Lake and Sussex Inlet. Inland by arrangement.
