Privacy policy.
Supporting somebody means holding information about them that is nobody else’s business. Here is exactly what we collect, why we need it, who ever sees it, and how you get it back or get it fixed.
Who we are
Care Awareness South Coast Pty Ltd, ABN 57 744 537 889, is a registered NDIS provider based at Narrawallee, New South Wales. We also support aged care Home Care Package clients, DVA clients, and private clients across the Shoalhaven and south to Batemans Bay.
We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and by the NDIS Code of Conduct.
What we collect
From participants and clients: name, contact details, date of birth, NDIS or Home Care Package or DVA reference, plan details and funding, the supports you want, and the health and disability information we need to deliver those supports safely. That can include diagnoses, medications, allergies, mobility and communication needs, behaviour support plans and incident records.
From families, carers, nominees and support coordinators: name, contact details and your relationship to the person you are enquiring about.
From people applying to work with us: name, contact details, availability, qualifications, checks and clearances, and work history.
From anyone using this website: what you type into an enquiry form. We do not run advertising or tracking cookies on this site.
Why we collect it
To answer your enquiry, to work out whether we are the right provider for you, to deliver and roster supports, to keep you and our workers safe, to invoice the NDIA or your plan manager or you, and to meet our record-keeping obligations as a registered provider.
We collect health information only where you consent and where we need it to provide a support safely. You can decline, and we will tell you plainly if that means we cannot deliver something.
Who we share it with
Only where you have agreed, or where the law requires it. That usually means your support coordinator or plan manager, your allied health team or treating practitioners, the NDIA, the NDIS Quality and Safeguards Commission, or an aged care or DVA body where your funding sits with them.
We disclose without your consent only where we are required or permitted by law, or where there is a serious and imminent risk to somebody's life, health or safety.
We do not sell personal information, and we do not disclose it to anyone overseas.
How we store it
Records are kept for as long as we are required to keep them, which for NDIS participant records is generally seven years, then destroyed or de-identified.
We keep records in physical and digital form, restrict access to the people who need it to do their job, and use password protection on the devices and accounts that hold them.
If a data breach is likely to cause you serious harm, we will notify you and the Office of the Australian Information Commissioner, as the Notifiable Data Breaches scheme requires.
Getting access, or correcting something
You can ask for a copy of the personal information we hold about you, and you can ask us to correct anything that is wrong or out of date. Ring 0436 280 272 or email support@careawareness.com.au.
We will respond within 30 days. There is no charge for asking. In the rare cases where we cannot give you access, we will tell you why in writing.
If you are not happy
Tell us first. Ring 0436 280 272 or email support@careawareness.com.au and we will acknowledge it within five business days and work through it with you. Raising a complaint never affects the supports you receive.
If we have not resolved it, you can go to the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992. For anything about the quality or safety of your supports, you can contact the NDIS Quality and Safeguards Commission at 1800 035 544.
